Contractures alone can be a co-factor in the development of hemiplegic shoulder pain (shoulder joint pain). Approximately 16% -72% of stroke patients experience hemiplegic shoulder pain. and the incidence rate can increase to more than 80% in stroke patients with hemiplegia in which the upper arm has minimal voluntary movement or no voluntary movement at all. Contractures and hemiplegic shoulder pain often prevent patients from participating in rehabilitation programs, contributing to poor outcomes of upper limb function and decreasing patient independence in ADLs. Purpose – To apply the arm positioning program to preventing the occurrence of shoulder abduction and external rotation contractures shoulder and hemiplegic shoulder pain in stroke patients with hemiplegia.Method – this EBN implementation using Quasi Experiment pre design and post test design. The number of samples in this EBN implementation is 15 respondents treated in the inpatient room according to the inclusion criteria. Evaluation done on the first day and last day (3 weeks). Results – Results obtained after the positioning program at the end of the 3rd week, got the average score of shoulder abduction ROM value in the normal range is 180o, the average score of ROM value the external rotation of the shoulder is also within the normal range of 90o and no evidence of shoulder joint pain is observed. It appears that no decrease in shoulder joint motion, especially shoulder abdominal ROM and external rotation of the shoulder and absence of joint pain from before and after positioning Discussion – After a stroke, weakness of the upper limb muscles will cause the patient having difficulty moving the arm to move away from the side the body as a result of impaired motor functions of the body. The patient arm will be in imobit condition with the internal shoulder rotator muscles, adductor and extensor in short position / condition. Muscles or tendons in the human body will experience a permanent shortening in response to excessive hypertonic stress that occurs continuously in the muscle or tendon, such as a spontaneous spasticity / rigidity. Recommendation – With the application of an upper arm positioning program can prevent the occurrence of external abduction of shoulder abduction shown in the absence of a decrease in shoulder joint range for abduction and external rotation of the shoulder. In addition, this positioning program can also prevent the occurrence of shoulder joint pain that often develops simultaneously.
