ABSTRACT

Preliminary. Hypertension is still an important problem in old age. Education and adherence to taking antihypertensive medication is one of the factors that are part of the management of hypertension in a holistic and comprehensive manner. This study was conducted to assess the effect of structured education and adherence to taking antihypertensive drugs to decrease blood pressure in elderly patients. Methods. Double blind randomized clinical trials in October 2012-February 2013 were carried out on elderly patients with hypertension in three polyclinics at Dr. Cipto Mangunkusumo (RSCM), Jakarta. Subjects were divided into 3 groups, namely group I who received structured education and checklist, group II who received structured education and group III without structured education with a checklist. Structured education and checklists are given 3 times per month for 90 days. Anova analysis was performed to see differences in blood pressure in the three groups after intervention with the principle of analysis per protocol. Results. A total of 182 subjects met the study criteria and followed the study until the end, which consisted of 60 subjects in group I, 61 group II subjects and 61 group III subjects. At the end of the observation, systolic blood pressure (TDS) groups I, II and II decreased respectively to be 130 (range 90-179) mmHg, 135 (range 80-174) mmHg and 133 (range 102-209) mmHg ( p = 0.04). Meanwhile, diastolic blood pressure (TDD) groups I, II and III respectively decreased to 70 (range 48-100) mmHg, 74 (range 45-103) mmHg and 78 (range 60- 102) mmHg (p < 0.001). Conclusion. Structured education had a significant effect on the decrease in systolic and diastolic blood pressure in the elderly hypertension group, whereas the adherence to taking antihypertensive drugs in checklist did not have a significant effect.
